Your spouse is not being lazy, dramatic, or selfish. Depression involves real changes in how the brain works, along with genetic, biological, and life-stress factors, and this can make everyday tasks feel overwhelming and drain the pleasure out of activities they once enjoyed. When you truly understand this, your empathy and patience will come more naturally.

Be present without trying to fix everything. One of the most common mistakes I see well-meaning partners make is jumping into problem-solving mode. Sometimes your spouse does not need solutions. They need to feel heard and understood. Sitting with them, listening without judgment, and saying something as simple as I am here for you and I am not going anywhere can be more powerful than any advice.

Encourage treatment without being pushy. Gently suggest that they speak with a professional, offer to help them make the appointment, or even offer to come along for the first visit. If they are already in treatment, support their medication adherence and therapy attendance. Do not suggest they just snap out of it or try harder. These well-intentioned comments can feel devastating to someone struggling with depression.

Take care of yourself. This is not selfish. It is essential. Supporting a depressed partner is emotionally exhausting, and you cannot pour from an empty cup. Maintain your own social connections, exercise, and consider speaking with a therapist yourself. Caregiver burnout is real, and addressing it helps both you and your spouse.

Watch for warning signs that the depression is worsening, including talk of hopelessness, giving away possessions, increased isolation, or any mention of suicide. Without knowing about the diagnosis, teachers may misinterpret your child's struggles as laziness, defiance, or lack of intelligence, none of which are true. Sharing the diagnosis reframes the narrative and opens the door to the support your child deserves.

Once the school is aware of the diagnosis, your child may be eligible for formal accommodations under a 504 Plan or an Individualized Education Program. These can include extended time on tests, preferential seating near the teacher, written rather than verbal instructions, breaks during long tasks, reduced homework loads, and many other strategies that level the playing field. I have seen these accommodations transform a child's school experience from one of constant failure and frustration to one of genuine success and confidence.

I recommend starting with the school counselor or psychologist and your child's primary teacher. You do not need to share the full details of your child's treatment, only that they have been diagnosed with ADHD and what accommodations would be most helpful. Some parents worry about stigma, and I understand that concern. However, in my 35+ years of practice, I have found that the benefits of school support far outweigh the risks. ADHD has one of the strongest genetic components of any psychiatric condition. Research consistently shows that ADHD runs in families, with heritability estimates of approximately 75 to 80 percent. To put that in perspective, ADHD is about as heritable as height. So yes, if your child has been diagnosed with ADHD, there is a meaningful chance that you or your spouse may have it as well.

In my practice, I see this pattern play out regularly. A parent brings their child in for an ADHD evaluation, and as we discuss the child's symptoms, the parent starts recognizing those same patterns in their own life. They might say something like wait, I do all of those things too, or I just thought that was normal because I have always been that way. These moments of recognition can be genuinely transformative.

Here are some signs that you might benefit from your own evaluation. Do you have difficulty staying organized despite your best efforts? Do you frequently lose track of time, miss deadlines, or forget appointments? Do you start many projects but have trouble finishing them? Do you feel like you are working much harder than others to accomplish the same tasks? Were you ever told as a child that you were smart but not living up to your potential? Do you feel restless, have trouble sitting through long meetings, or tend to fidget? Any combination of these could suggest adult ADHD.

Many adults have developed sophisticated coping mechanisms that mask their ADHD symptoms. They might use extensive lists, alarms, and reminders, or they might rely on a highly organized spouse to keep the household running. These strategies can be effective enough that the ADHD goes unnoticed for decades, but they often come at the cost of constant mental effort and internal stress.

I strongly encourage parents of children with ADHD to consider their own evaluation. Getting diagnosed and treated as an adult can improve your work performance, your relationships, your self-esteem, and importantly, your ability to support your child who is dealing with the same condition. Some do not believe anything is wrong with them, especially if their condition was diagnosed by others rather than through their own recognition of a problem. Others are dealing with side effects they have not told you about, such as feeling flat, gaining weight, or experiencing changes they find embarrassing. Many teens resent the idea that they need a pill to be normal, particularly when they are in the developmental stage of establishing independence and identity. Peer concerns also play a role. They may worry about being seen taking medication at school or being different from their friends.

Here is what I recommend. First, have an open and non-confrontational conversation. Rather than lecturing, ask genuine questions. What bothers you about taking the medication? How does it make you feel? What would make this easier for you? Listen without judgment. Their feelings are valid even if you disagree with their conclusion.

Second, involve them in their treatment decisions. Teenagers are much more likely to follow through when they feel they have some control over the process. Bring them to appointments, let them ask me questions directly, and let them know that their preferences matter. Sometimes adjusting the medication form, such as switching from a pill to a long-acting capsule, or changing the timing of the dose, makes a real difference.

Third, do not turn medication into a power struggle. Forcing medication on an unwilling teenager almost always backfires. They may pretend to take it while secretly disposing of it, or the conflict may damage your relationship to the point where they refuse all help entirely.

If medication truly is not going to happen right now, we can explore other options. Therapy, behavioral strategies, exercise, sleep optimization, and school accommodations can all play important roles. Sometimes stepping back from the medication fight and focusing on building trust opens the door for the teenager to reconsider medication later on their own terms.

I encourage you to bring your teenager in so we can have an honest three-way conversation. Often, hearing the rationale from a doctor rather than a parent makes a difference.

In my experience, children who understand their diagnosis cope better, cooperate more with treatment, and develop healthier self-esteem than those who are kept in the dark.

The key is using language that matches your child's developmental level and framing the diagnosis in a way that is honest, hopeful, and free of stigma. Here is the approach I recommend to the families in my practice.

Start by normalizing the situation. Children as young as five or six can understand the concept that everyone's brain works a little differently. You might say something like you know how some kids need glasses because their eyes work differently? Well, your brain works a little differently too, and that is okay. It is not good or bad, it is just how you are made. We are going to learn about it so we can help you feel your best.

For ADHD specifically, I often suggest explaining it as having a brain with a really powerful motor. Your brain has so many ideas and so much energy that sometimes it is hard to slow down and focus on one thing at a time. The medicine helps your brain use that energy in a way that feels better for you.

For anxiety, you might describe it as having a worry alarm in your brain that goes off too easily. Most people have a worry alarm that only rings when there is real danger, but yours rings even when things are actually safe. We are going to learn how to turn down that alarm.

Avoid using the diagnosis as an excuse for behavior, but do use it as an explanation. There is an important difference. Saying your ADHD made you do it teaches helplessness. Saying I know it is harder for you to wait your turn because of how your brain works, and we are practicing ways to make it easier teaches understanding and empowerment.

Be prepared for follow-up questions, and do not be afraid to say I do not know but we will find out together. Answer honestly, keep it simple, and always circle back to the message that they are loved, supported, and not alone.
