What to expect at your first visit
The Specialty

When standard treatments haven’t fully worked,the answer is usually a more careful look.

Many people I see have already been treated by a primary care doctor or family physician who did the right thing for what they saw — tried Prozac, Paxil, Wellbutrin, or one of the standard options. Sometimes it works for a year and then stops. Sometimes the side effects make it untenable. Sometimes the original diagnosis only captured part of what’s actually going on.

About one in three people don’t fully respond to first-line treatment — and that’s not your fault. When two or more medications haven’t worked, we call the condition treatment-resistant, and the work shifts: re-examining the diagnosis, looking at every symptom that hasn’t fully cleared, and adjusting carefully every month until things actually feel right. No matter what brings you to our practice—whether you think you have ADHD, anxiety, depression, OCD, or you're not sure what's wrong—our evaluation process is the same. We don't just look for what you think you might have. We evaluate for everything.

Why? Because many patients who come to us thinking they have one condition actually have two, three, or more. Anxiety can look like ADHD. Depression can mask OCD. Getting the complete picture changes everything about your treatment.

For Adults

Approximately 2 Hours Total
  • One-hour comprehensive assessment with our licensed professional counselor
  • Counselor extensively reviews findings with Dr. Shapiro (he takes detailed notes)
  • One-hour meeting directly with Dr. Shapiro to review findings, clarify details, and discuss diagnosis
  • All treatment options explained—you decide together what feels right
  • Regular follow-up appointments, with the plan adjusted over time, working toward the best response we can achieve together

For Children & Teenagers

Approximately 3 Hours Total
  • One-hour meeting between parents and our licensed counselor
  • Separate one-hour session where counselor meets with your child
  • Rating scales collected from parents, teachers, and sometimes the child
  • Counselor looks for everything: ADHD, anxiety, depression, OCD, mood disorders, learning differences
  • Counselor extensively reviews all findings with Dr. Shapiro
  • One-hour family meeting with Dr. Shapiro (parents AND child attend together)
  • Child is included in treatment decisions
  • Regular follow-up appointments, with the plan adjusted over time, working toward the best response we can achieve together

When You Call Our Office

What new patients hear from us first

Before any appointment is scheduled, here's exactly what we'll explain about how our practice works — so you can decide if we're the right fit.

A two-clinician team that shares one chart

Our therapist and Dr. Shapiro work as a team and share one record. When you have a therapy session, Dr. Shapiro sees those notes before your next medication visit — and the other way around. You never have to repeat your story.

24-hour callbacks. Full evaluation done in 10 days.

We're a small practice on purpose. Our therapist gets back to you within 24 hours, and we aim to complete the entire evaluation within about ten days, working hard to make that happen as quickly as your schedule allows. Larger practices often can't do that — we can.

We bill your insurance for you

We're out-of-network, but as a courtesy we still submit your claims to your insurance company twice a month. Most out-of-network practices hand you a superbill and leave the paperwork to you. We don't.

If our fees would add real stress, we'll say so

We genuinely want what's best for you and your family. If our fees would add real financial stress, we'll tell you honestly and help you think about finding someone in network. There's no pressure to use our practice.

No pressure to decide on the spot

Take the information home. Talk it over with your spouse or family. When you're ready, call back — you don't even have to go through intake again. We'll be here.

"This is one of the most common questions I hear—especially from men who were raised to 'tough it out.'"

Here's what I tell them: You don't NEED medicine. But your life could be easier, better, more enjoyable. Medicine is a tool, not a crutch. And you're always in control of whether to use it.

My Rule for Medications:

If a new medication doesn't feel right, give it a couple of days, then call us right away — don't stop on your own, since some medications need to be tapered. We'll guide you on the safest next step and figure out what happened.

My commitment is simple: we keep working together and adjust the plan as needed to help you feel as well as possible. If one approach doesn't work, we try another — and we'll keep exploring reasonable options together.
